A fee of $13 per mailed in surname request and $15 per faxed in request is required, with faxed in requests requiring payment in the form of a credit card. These checks are for records in Iowa only and are based on name and date of birth. Without fingerprints a positive match cannot be assured. Only the Division of Criminal Investigation can perform such checks and disseminate them to non-criminal justice agencies and requesting individuals.
