Court Record Resources
To request a criminal history record information check, a form must be completed and submitted along with a fee for processing the request. The request form must include:
- The name of the requesting individual or agency
- The name of the subject individual
- The subject's current address
Along with two additional identifying markers from this list:
- The subject's fingerprints
- The subject's social security number
- The subject's date of birth
- The date of a specific reportable event such as arrest or conviction
The majority of criminal history record checks are done through the use of name, date of birth and social security identification number, but these are not considered legally complete checks without the verification of fingerprint comparison as well as checking any maiden names, aliases or former names.
